HealthPoint Practices Receive National Recognition for Patient-Centered Care
 
   Patient-Centered Medical Home 2011 standards emphasize enhanced care through patient-clinician partnership

Tampa, February 12, 2013:  Several HealthPoint Medical Group practices have received recognition by the National Committee for Quality Assurance (NCQA) under the Patient-Centered Medical Home 2011 Recognition program. The Internal/Family Medicine locations that received the PCMH 2011 Recognition include: Apollo Beach Blvd., Bearss Ave., Dr. Lee Kirkman's practice at St. Joseph's Hospital on Dr. MLK, Jr. Blvd., Race Track Rd., Premier North Dr., South Church Ave. (Adult), Van Dyke Rd. (Adult) in Lutz, Walden Lake (Adult) in Plant City, West Linebaugh Ave. (Adult) in Westchase, and St. Joseph's Community Care on West Waters Ave.

The program recognizes practices (also known as "medical homes") that use evidence-based, patient-centered processes.  The benchmarks of patient-centered care include open scheduling, expanded hours and appropriate use of proven health information systems. Each patient's care is tended to by a clinician-led care team. This team provides for all the patient's health care needs and coordinates treatments across the health care system. Research shows that medical homes can lead to higher quality and lower costs. They can also improve patients' and providers' experiences of care.
